FINANCIAL ASSISTANCE PROGRAM (FAP)

DISN has developed a program designed to meet the needs of individuals without health insurance. We recognize that if you have no insurance, you need a place to go to get quality diagnostic imaging without having to pay full cost.  The program provides for a sliding scale of payment based on family size and current level of income.

If you think you may be qualified for the FAP program call us at 433-6100, ask for scheduling, and mention the FAP program.  Our schedulers will be happy to discuss the program with you.  Or you can come to DISN's facility at 3560 E. Flamingo, Suite 100, to discuss the FAP program in person.
